NDIS PLAN MANAGEMENT SERVICES

Wellington Care provides reliable Plan Management services to help participants manage their NDIS funding with ease and confidence.

Our team handles the financial administration of NDIS plans, allowing participants to focus on achieving their goals and accessing the supports they need. 

Our Plan Management support include:

THERAPEUTIC SUPPORTS – PSYCHOLOGY & COUNSELLING

Wellington Care provides professional therapeutic supports to improve the emotional wellbeing, independence, and quality of life of NDIS participants. Our AHPRA-certified therapists deliver personalised psychology and counselling services to help individuals manage challenges, build resilience, and achieve their NDIS goals.

Our Therapeutic Supports may include:
Complex  Psychology assessments and therapeutic interventions
Complex  Individual counselling and emotional wellbeing support
Complex  Support for anxiety, stress, depression, and mental health challenges
Complex  Behavioural therapy and positive behaviour support strategies

GROUP- BASED ACTIVITIES

Our Groups provide activities to help people with disability to live a healthier, more rewarding and socially engaged life. Wellington Care Community Health Services provide a program of activities that maintain and enhance the daily living skills of all participants. The program of activities addresses a participant’s physical, social, cultural, emotional and recreational needs and provides opportunities to stay connected to the community.

DEVELOPMENT OF LIFE SKILLS SERVICES

Wellington Care Community Health Services staff prioritise the fostering of existing skills and development of new skills to facilitate our clients to enjoy the opportunities available to them. We acknowledge that a person’s skillset is crucial to their sense of identity. MCHS will work with clients to best utilize their skills in accomplishing meaningful goals and health outcomes. New skills can be introduced to enhance the quality of life experienced by our clients, opening opportunities in independence and contributing to family life at home.

Wellington Care Community Health Services utilizes a multidisciplinary team to document and track the achievement of client goals as guided by the National Disability Insurance Agency. Dedicated management works with the clients, support workers to determine best practices in developing new skills and introduction of new skills.

IN HOME SUPPORT / PERSONAL CARE

Our trained staff can help you learn the skills you need for a variety of day-to-day activities from the comfort of your home. Wellington Care community health services can also provide this support to people with complex needs, provided it has been approved for this service. Personal activities can be:

COMMUNITY PARTICIPATION

Our community participation service for NDIS participants is designed to help individuals with disabilities become involved in their local communities, develop new skills and interests, build friendships and connections, and improve their overall quality of life. We strive to provide a person-centred approach to community participation, ensuring that each individual’s unique needs, preferences, and goals are met. .

 

Our respected staff are available to facilitate participants’ access to their community confidently. We support participants with personal grooming, travel training, role modelling, communication and navigation, so they can look their best and feel their best to help them get the most out of their life in the community

ASSIST IN TRAVEL/TRANSPORT

medical  Transport to appointments: We can provide transport to medical appointments, therapy appointments, or other appointments as required.
community  Community access: We can assist with transport to community facilities and services, such as libraries, community centres, and public transport.
social  Social and recreational activities: We can provide transport to social and recreational activities, such as sports events, concerts, or shopping trips.
travel purposes  Airport transfers: We can provide transport to and from airports for travel purposes.
public transport  Assistance with public transport: We can assist with using public transport, such as trains, buses, or trams.

ASSISTIVE EQUIPMENT & MOBILITY SUPPORT

Wellington Care helps NDIS participants live more safely and independently by supporting access to high-quality assistive equipment. We work closely with participants, families, and healthcare professionals to identify equipment that improves daily living, mobility, and community participation.

 

Our team provides guidance throughout the process to ensure the right equipment is selected based on each participant’s needs, goals, and NDIS plan, helping improve safety at home and support a more active lifestyle.


Examples of assistive products that may be supplied or arranged include:

Complex  Wheelchairs, walkers, and mobility scooters
